I have voluntarily chosen to participate in this MYCamp at facilities provided by Ilero United Muslim Association. I acknowledge and accept that  MYCamp  may involve certain conditions, hazards and risks, including injury or death or damage to property, and that  MYCamp  cannot control these risks. I acknowledge there may be physically strenuous activities and certify that I am fit and physically and mentally capable of participation in all activities provided by  MYCamp. I have disclosed any and all medical conditions that could impact my own safety, or the safety of others. I understand that I am responsible for administering all required medication to myself and  MYCamp will not administer medication to me. I certify that I am liable to cover any personal injury which may be sustained while participating in this camp. In consideration of  MYCamp providing the opportunity for me to participate in this camp, I release Ilero United Muslim Association, its Board of Trustees, officers, employees, and representatives from any and all liability to me, my parents and/or legal guardians, our personal representatives, estate, heirs, and assigns for any and all claims, demands and causes of action for any and all illness or injury to me, including death, and property damage arising out of, during or in any way connected with this camp. I agree to indemnify and hold harmless, waive and covenant not to sue Ilero United Muslim Association, its Board of Trustees, officers, employees, and representatives from any liability for the injury or death of any person(s) or damage to property that may result from my negligent or intentional act or omission while participating in the camp. I hereby authorize the staff of  MYCamp to act for me according to their best judgment in any emergency requiring medical attention. I authorize and give consent for  MYCamp to administer general first aid for any minor injuries or illnesses experienced by me during camp participation. If I am in need of emergency medical care and  MYCamp is not able to reach my parent or the emergency contact, I authorize  MYCamp to sign all necessary papers and arrange for emergency treatment and hospital care. I understand that  MYCamp is not responsible for any medical expenses associated with any personal injury I may sustain and understand that  MYCamp does not provide medical insurance/Coverage for me.
